Heart of Bath Medical Partnership are currently accepting new patient registrations. We would encourage patients to register online (see below form) during the current pandemic, if this is not convenient you can attend the surgery to collect paper registration forms ensuring you follow the guidelines with social distancing and wear a face covering.

Please note the following:

  • We cannot register you without your NHS number.

Please make sure you include your NHS number on this form – without it we will not process your registration. If you do not know your NHS number please contact your previous/current GP surgery and they can give this to you. If this is your first time registering with the NHS please make sure to include your place of birth and first date of entry into the UK.

  • Please clearly complete every section/ box on the form.
  • Due to reduced staffing, registrations are currently taking up to 2 working weeks to process – if you have consented to SMS messages you will receive an SMS notification once you have been registered.

Heart of Bath do not accept new patient registrations outside our Practice Boundary. Practice Boundary post code checker is found on the home screen under Patient Info > Practice Boundary St James and Oldfield.
