We ask all our patients to keep their addresses and contact telephone numbers updated. You can do this by completing our online form.
If you have moved outside our practice boundary you may still remain a permanent patient with Heart of Bath but when we change your address you will have a registration status set as an ‘out of area patient’ this means you will not be entitled to home GP visit, if you need medical attention at home you will need to phone 111. If you have long term health worries it is suggested to register with a more local surgery. Please check your postcode in our practice boundary section (under Practice Info)
In some cases, if you move quite a distance away from the surgery but you work or stay in Bath and the surrounding areas the process to register you as an ‘out of area patient’ is slightly different, we will need to deduct you and re register you with the Health Authority, this can take up to 72 hours to process.
