Please note that every care has been taken in the preparation of information on this Postcode Mapping Tool, however there may be instances where the Postcode Mapping Tool does not include your location even though it is within our Practice Boundary. An example of this may be new housing developments where the postcodes are not yet included on Google.
NHS Out of Area Patient Scheme:
Heart of Bath Medical Partnership offers the NHS Out of Area Patient Scheme to accommodate patients who reside outside our practice catchment area* (see map above) but wish to register/ remain registered with our practice.
As an Out of Area (OOA) patient you will have access to our clinicians during our normal surgery hours, i.e. 08.00 to 18.00 Monday to Friday. However, you are NOT eligible for GP home visits. Upon registration with the OOA scheme you will be sent further information regarding what to do if you develop an urgent illness or injury that prevents you from attending our surgery.
If in the future you wish to register with a GP practice in your local area, please register with your chosen surgery (this will automatically start your removal from our practice). Your local practice will then take over your care.
Current Heart of Bath patients:
If you have been contacted regarding your out of area address and you wish to remain registered with HoB, you will need to be deducted and re-registered to be put on the Out of Area Scheme (this process takes approximately 7 days to be completed by the local Health Authority). Please ensure you have all repeat medication to cover this transition period.
The NHS Out of Area Scheme is a scheme we opt in to, this can be subject to change, which may result in you being asked to register locally in the future.
*Please note we accept/decline OOA registrations depending on the patient’s medical requirements and where their address is located (address must be within a reasonable distance from our practice).
